couldn't you use these? 

      National Number Prefix : This parameter defines the number that is prefixed to an incoming national number, and provides a means to help you identify national numbers, if necessary. This parameter allows you to prefix specified digits to the calling number of an inbound call on the basis of the Type of Number field in an inbound offered call (ISDN SETUP message). For example, a national call from 4085551212 could get prefix 91. 
      Maximum length: 8.
      Allowed values: Up to 8 digits (0-9,#,*).
     
      International Number Prefix : This parameter defines the number that is prefixed to an incoming international number, and provides a means to help you identify international numbers, if necessary. This parameter allows you to prefix specified digits to the calling number of an inbound call on the basis of the Type of Number field in an inbound offered call (ISDN SETUP message). For example, an international call from 456789 could get prefix 9 011. 
      Maximum length: 8.
      Allowed values: Up to 8 digits (0-9,#,*).
     
      Subscriber Number Prefix : This parameter defines the number that is prefixed to an incoming subscriber number, and provides a means to help you identify subscriber numbers, if necessary. This parameter allows you to prefix specified digits to the calling number of an inbound call on the basis of the Type of Number field in an inbound offered call (ISDN SETUP message). For example, a subscriber call from 1212 could get prefix 9. 
      Maximum length: 8.
      Allowed values: Up to 8 digits (0-9,#,*).
     
      Unknown Number Prefix : This parameter defines the number that is prefixed to an incoming unknown number, and provides a means to help you identify unknown number types, if necessary. This parameter allows you to prefix specified digits to the calling number of an inbound call on the basis of the Type of Number field in an inbound offered call (ISDN SETUP message). For example, an unknown call from 5550123 could get prefix 7. 
      Maximum length: 8.
      Allowed values: Up to 8 digits (0-9,#,*).

  I don't think there is a way in the CallManager to modify the number  
  sent after the original call setup.  If the gateway can do it I can't  
  say.

  Short story without the big nasty CCM trace snippets is that yes, the
  PSTN is sending a H225 Notify message with a ConnectedNumber IE.
  Once CCM receives this it sends the new calling party number out to
  the IP phone(s).

  You can start with Q931 translator to find the call ref that matches
  what is in your q931 trace.    Then search based on the calling party
  number and you should see it being sent in a StationD CallInfo
  message to the IP phone.

  connected.  Do you have a CCM trace of the call?

  Have you looked at a q931 debug on the router?  Most likely the PSTN
  is sending a Connected Number IE and CM is updating the call to show
  this.
