[cisco-voip] Perflib perfts.dll error and subsequent ccm.exe crash

Anyone ever have this before on a CCM server?
4.1(3)sr2 and OS 2000.4-2sr2.
After the following error, the ccm service terminated
unexpectedly and would not restart by itself. It would
not restart by itself because the IP Voice Media
streaming service was pegging the CPU at 100%.
