[cisco-voip] using t1 ground start for LD

we are going from meridian to cisco.  currently they use t1 ground start for
LD.  I know this a old technolgy but need to configure the voice gateway to
handle it. we have 2821 running 12.4.5a.  we are using vwic ti controllers
