[cisco-voip] Software MTP will not register

Can not get an MTP to register with CCM  4.1.3sr5d.  Router IOS is 12.4.11T.  Need the MTP to provide Hold for phones using SIP trunking thru a IPIPGW. I have made sure the name in the router and CCM match. What am I missing? 
 
Getting these errors in debug:
