[cisco-voip] CFA to predefined number

While a very good idea, unfortunately CM doesn't use the CFA CSS when  
doing DA on the CFA destination so tricking it with a plar won't work.

The easiest way to do this is going to be to design a service that  
uses AXL to update the CFA destination for the phone and then assign  
this service URL to a spare line button on the phone.
