[cisco-voip] Fractional PRI with "ccm-manager config"

I have some sites where the MGCP controlled b-channel is busied out for fractional. Not supported but it works.

May be able to hack it to work by busying out the unused channels from a 
callmanager perspective through the maintenance service parameter, but 
definitely a hack.
