I ran into that once here an outside caller could not dial the DN or find the person based on name spelling. I finally just deleted the VM box and recreated it. That resolved the issue. It only effected one person in the AA.

  If you can't find a commonality with the problem calls, you're really just going to be guessing at things. You could setup traces on callmanager. Skinny DTMF digits are pretty easy to filter out, it'll be a KeypadButton message. That way you'll at least be able to tell if the digits are making it to callmanager.

      Is your gateway h323 or MGCP?   

      Just verifying the basics, but you have dtmf-relay h245-alphanumeric on your dial-peer, or mgcp dtmf-relay voip codec all mode out-of-band for your mgcp config?  I've seen sporadic dtmf input issues on an IVR when either of these were missing from their respective configurations.

      I've had a number of situations where a caller is not able to enter an extension at our Unity AA. Most of the time it's with older PBX's but they say it works with other auto-attendants.

      Has anyone come across any reason why this would happen?

      I doubt it's a PRI issue since we'd have more than just a few calls. And those calls would have to always come in on the same trunk.
