On the IOS version, I believe the issue is more the SRST version, which
must be a minimum of SRST 4.1 for UCM 6.1(2).

I've never heard this regarding the IOS version but I guess it's
possible. 12.4(15) is terrible, particulary when it comes to the MGCP
process getting hung so lets hope this isn't the case.
I've seen this in the past, as Ryan mentioned, how are the ccm servers
referenced under servers? Domain name or ip address? I would also remove
ccm-config if you can, the ip host entry is a good band aid. It seems
like once you do get it set the way you want it and ccm-config is on
there, it will put the reference back in as the domain name.
Also be careful when removing ccm-config, it tends to crash/reload the
router about 5-10 times so do it a maint window if it's crucial.

No ip hosts are configured in the router.
Although I thought about adding a "ip host lonccm1 192.168.174.10" if I
can't fix this!
After typing this I went ahead and added the ip host entry in IOS and
ccm-manager registered. I'll keep the workaround in place.
