[cisco-voip] CME, operator unable to transfer a call coming from AA script

026731XXXX calls 100, is redirected to 136 (Operator), who in turn  
tries to forward the call to 111 but fails.

ephone detail, voice ccapi inout and isdn q931 debugs were enabled.  
Although the  failed call should be included from beginning to the  
end, it is possible that other, internal calls were partially captured  
as well.
