One issue with this is having to ensure that additional components are always operating. Things go down, whether it's Unity or IPCC, etc. Then there's maintenance. 

I like keeping things internal.

A solution like this that I came up with was an AA which said, "If this 
is a 911 emergency, press * now to be connected with 911, or remain on 
the line. If this is not an emergency hang up now." and set only * to 
dial forward the call to 911, and after 5 seconds of no key is pressed 
to send the call to 911. 

There are some sites and situations where 911 calls must be routed 
instantly for maximum safety (where it may be hung up on by an attacker) 
and/or at the same time you want to alert key personnel to know that the 
call has been is placed so emergency responders or on-site security can 
be directed to the exact location. Syn-Apps's Call Alert is perfect for 
this:

For instance at a school or large campus, an email and XML screen pop 
can be sent to the front desk, the school/building administrator, and 
the security personnel. That way when 911 calls back on a hang-up, the 
front desk knows where specifically the incident is, etc. 

Using Unity (or whatever other method), you could have the email trigger 
a phone call if it is not marked read within a minute, etc.
