I think many share you point of view. I think the original question was referring to a new install? 

We don't upgrade unless we have to or there is a new feature we require or a bug fix. For many of the reasons you describe.

IMHO I always err towards the “if it ain’t broke, don’t fix it” school of thought. If I’m on a version that has all of the features I need and is stable and functioning properly (for me) and I feel it is secured properly (firewalled off from the Internets), I won’t rush out to apply the latest release the day after it is posted. I typically would recommend applying the “latest and greatest” stable version (a version that is at least a week old) usually quarterly, simply to keep from getting too far behind. I stay away from the “bleeding edge” releases. Cisco is notorious for releasing a “12.x(x)” code and then three or four days later releasing a “12.x(x)a” code that fixes a bunch of bugs in the first release. 



Having said all of that, if I have any major issues I’ll check the latest code for bug fixes related to my issues and typically try the new code before calling TAC.

IMHO this is very critical. If you come to me for support I cannot give you a "minimal set" change between where you are and where the fix is available. You are going to get all of the changes from 6.1.3 to 6.1.4 in addition to any new fix that is tailored to your environment. I may be a bit biased as my business leads me to expect things to be broken. To that end it appears prudent to get as close as possible to any patches that may be necessary. 

I would very much like to hear the positions of others.

Ah, yes, that's another way to look at it. As each dot.dot release is released, the previous likely gets shelved for development.

6.1(3) is "done" meaning no more ES will be built for it. If you upgrade to 6.1.3 and encounter a serious issue Cisco will require you to upgrade to 6.1.4 or an engineering special of 6.1.4 to get a fix. 

net: 6.1(4) is the "best" place to be currently.
