Have you confirmed that both MSFC's are properly seated on the
Supervisor, and visible from CatOS (ie "show mod [15|16]", "session
[15|16]", etc)? This is sometimes an indication that they're not.
Unless you're handy with tools, I'd suggest trying to trade what you
have for a pre-assembled Sup+PFC+MSFC. Seating (and more importantly,
fastening) the MSFC is a bit more difficult than one would expect.
