[nsp] CISCO 4000 or 4000 M or 4500

I have a chassis of CISCO AXE
and I have no clue if this is a 4000 or 4000M or 4500
or similar.

I went trough the DOCS on CISCO site but could not find answer.
Can somebody help me with this? How to recognize the chassis?

If this can help:

cisco 4000 (68030) processor (revision 0xC0) with 8192K/4096K bytes of
memory.
Processor board ID 5028944
