[nsp] 3620 gone bad

Hello,

I awoke to pages this morning and found a 3620 (NM-1E-2W + 2 T1 WICS,
NM-2-CT1) of ours stuck in a loop:

Over and over, and sending a break would not stop it... Power cycling a
number of times let it boot OK, but after about 10 minutes of sitting,
this error starts appearing and then we enter the ROM loop as seen above:

%OIRINT: OIR Event has occurred oir_ctrl 10 oir_stat CFCD
and
%OIRINT: OIR Event has occurred oir_ctrl 10 oir_stat CFCC

This box has been running the same IOS 11.2(15)P for a few years, so I
don't think I'm suddenly tickling a bug, especially considering whatever
is wrong messes up the boot rom as well...

We're in the process of seeing if all card combos cause this or not.

Any ideas? The box is out of warranty, but I'm not sure which component
to blame...

Here's more info from boot. This is as it appeared on my terminal:
