So you definitely have reloaded and they are still showing up as bad?
What IOS/modem version are you running? I would open a TAC case and be
prepared to send them "sh tech modem"(or whatever)...possibly you need a
new carrier card.

Subject: [cisco-nas] Bad modems on AS 5300
Hello,
I have 12 modems on an AS 5300 that are newly marked as bad. How can I
troubleshoot further or remove them from the modem pool?
I have tried resetting one of them "clear modem 1/0" but it did not
help.
