Here's the general approach:

I.e. first prove that the E1 between the router and the switch in 
GENERAL works reliably.  Then find out what's going on wrong with the 
INDIVIDUAL calls.

Your modem call shows one (basically) good call on modem 0/2, followed 
by another call that failed to train.  What was different about these 
calls - different client modems?  Different client-side POTS lines?  
Different circuit path through your TDM network?  Different E1?  
Different DS0 used within the E1?
