We ran into this several times when we were still using hybrid mode.

It seemed to occur on all 6.x Cat OS trains. (Never tried 7.x)

Cisco had no answers except to reboot.  They did suggest
  no mls ip unicast

But that just caused the crash and rebooted for us.

My recommendation is to go native mode. We've never had anywhere near 
the problems we had with hybrid.
