Works for me, 7204VXR/225, 12.2(8)T5:

Slot 1:
        T3+ PA Port adapter, 1 port
        Port adapter is analyzed 
        Port adapter insertion time 20w0d ago
        EEPROM contents at hardware discovery:
        Hardware revision 3.0           Board revision A0
        Serial number     28675313      Part number    73-3762-03
        FRU Part Number:  PA-T3+= Serial
