Thomas - I think you might have been going with a bandwidth points approach. This looks more like a firmware problem on the port adapter itself.
Brian - If you have a SMARTnet contract, I would open a TAC case. I don't think there is much you can do on your own here.

128MB memory
20MB flash
C7200-I/O
tried the PA-MC-T3 in all four slots with same results.
