This seems to happen quite frequently in recent documents - "someone" is
writing a document using all the acronyms, and "someone else" is editing
it to include what they think is the correct explicit form for the 
acronym.

... and sometimes "they" guess wrong.

Otherwise there is no way to explain things like this in otherwise
perfectly reasonable documents...
