[c-nsp] 6k Sup720 Failover codes

[LUCAS, Raymond, FM]

Can anyone provide a description of the 'reason' and 'action' codes in the
output of "show log"?  Some samples provided below.

CCO has nothing that I can see.  Output interpreter shows nothing of
interest either.

These are from a couple of 6509s with 720s.
