[c-nsp] 6748 dead again

Another ws-x6748-ge-tx died on a different 6509, dual sup720's.  This time 
slot 3, not 4...  Same code, 8.3.5 on the catOS side.  Dual 4000 watt power. 
  This is the 3rd dead 6748 in about 3-4 weeks.  There are at least 20 other 
6748 cards running with the exact same hardware, but 8.2.2 code.  TAC case 
open...etc.  Any other users seeing something like this?  The actual
