Looks like the power to the module was turned off, do you have AAA on your catalyst logging commands?
A cat will continually reboot blades if it does not have enough power to run everything in it.

We're trying to troubleshoot why the FWSM in our 6513 just rebooted. So
far, we haven't been able to determine a reason. I have no idea if it's
a bug or a hardware issue at this point so I thought I'd check to see if
anyone here had any ideas. Here are the logs from the 6513:

And that's it. The FWSM doesn't provide any useful information that we
can find, e.g. reason for last reload.
The 6513 is running 12.1(23)E2 and the FWSM is running 1.1(3).
Any thoughts?
