First off, thanks to everyone for their suggestions and help.

Unfortunately not, after both swapping the cards (with a known good
one) and changing slots in the chassis I was still getting the error.
Interestingly enough the PA-FE-TX doesn't seem to be having any
problems in the chassis with any of the slots.  I have even tried a
different NPE, so the only common components are the chassis, the I/O
processor and power supply.  I think the next step for me is going to
be swapping out the chassis.
