Look here:
%SYS-5-RELOAD: Reload requested by LWAPP CLIENT. Reload Reason: INCOMPATIBLE
CONTROLLER VERSION.

You should upgrade to 4.2 as 4.0 will not support that AP.

Try 4.2.130.  Read the notes to go up to it.  Also note that as you upgrade, 
if you have any APs registered they will upgrade themselves.  So, plan 
appropriate downtime.
