[c-nsp] Multiple power supply failures. Advise needed

Out of the 7 failed PSUs 6 was original equipment. One cisco gear (AS5400) has power supply failed twice.
The 2nd failed power supply had been in service like 24-48 hours only before the failure, so may be it was just bad
power supply. I'm leaning toward metal particles build up as a most plausible theory at the point. 
I just don't know how to deal with the problem...
