[c-nsp] Switch 4500 module problem

We have 4500 series switch (WS-C4510R+E) in production. It has module
 10/100/1000BaseT Premium POE E Series.
On the module 13-24 ports are faulty. When connecting IP phones in these
range of ports then voice quality is poor and Ip phones frequently loosing
connectivity. Rest of all ports are working fine. I contacted Cisco and
they suggested to change module. I have changed module but still there is
same problem.  Configuration on the ports are similar on all ports of the
