Either your cables or transceivers are dirty.  You should buy yourself a 
CLETOP-S for cleaning your ferrules, and a pair of insertion cleaners for 
cleaning sockets (transceivers, patch panels, etc).  I use these ones, but 
there are several other manufacturers which make similar products:
