[c-nsp] Cisco 6509 / WS-CAC-6000W OUTPUT FAIL

I havent seen that exact behavior but I've seen reports of giving the 
front fan of the PSU a slight push to restart the whole thing. Do you 
have a spare chassis to test the PSU?

I had a 6000W fail with the OUTPUT ERROR where the front fan failed to 
start.
