[cisco-voip] Call Back failure

Dear Group,
I install CCM 3.3(4) with Service Release 2, and Unity 4.0(4),
all features are working except Call Back. When I push the
"CallBack" softbutton, the result is "Service is not Active".
I followed exact setup procedures for Call Back:

and checked around the FAQ:

actually "CTI Manager service" and "Extended Functions services" have been
activated.
I have no idea why Call Back is still not working, is there any tricky point
or missing point?
