Hi Scott,

Looks like AC or CTI Manager is not able to login with the username AC.  
Check AD to see if the account exists, then proceed thorugh is it locked 
out, etc.  Try to login through /ccmuser with AC and the default 
password 12345.

If this still does not work, delete and recreate the AC user as he may 
have lost some Cisco specific attributes.  Did you possibly change the 
Cisco OU on re-install?  The user object in AD has an attribute that 
says where to find the user profile, usually under the Cisco OU, and 
device association is stored there.  A sniffer trace with Network 
monitor will decode the login request to AD and response from AD to 
clarify what OU is being checked.  Then maybe check the event viewer on 
your AD server.
