The difference from internal/external is due to delay/variances in setup/progress/notify/alerting message exchange on the PSTN

That's odd. I'm not quite sure what would make that difference. I've tested trunk-to-trunk, IP-to-IP and external-to-UnityAA-to-IP and all behave the same way.
 
We're using 3.3(3)sr4a.
