[cisco-voip] Cisco phones trying to TFTP file ending in TLVextension

If you are on CM 3.x running 7.0 or 7.1 phone loads, the phone must receive
the TFTP File Not Found response from the TFTP server, otherwise it will
abort.  I recently encountered an issue with the PIX FWSM blocking this
message and preventing phones from upgrading CSCef67634.
