[cisco-voip] Attendant Console message "Initialization of Call Control Failed..."

The biggest thing to remember when dealing with AC is to create the user
"AC" in DC directory, and then add the phones that need call control to
"AC"'s controlled devices.  Without this, AC cannot access any phones.
