Two VERY silly suggestions - but both of them did happen to me:

1) I would try eliminating the cable as a potential cause (i.e. connecting
the offending phone to its switch port using a short known-to-be-good patch
cable). I had once one of those
'good-enough-to-turn-the-tester-light-on-but-not-to-connect' cables. (Gotta
love them.) Also, if you did not already, I would try swapping ports and/or
phones. (Meaning plugging a different phone into the offending switch port
and offending phone into another switch port.) 

2) My second occurrence of that lovely message was caused by hardware
failure of phone's built-in switch (it was a 7960, I think).

You got my hopes up briefly, but our failing phone's s/n is outside the 
range of those in the FN.  TAC asked for a bunch of debugging
information 
from the switch, but I expect an RMA phone replacement is in the wings.
