[cisco-voip] Sudden FXS problem

Had the same exact problem at 2 occasions. Everything was working
fine, then no outgoing. The first was fixed by upgrading the IOS on
the gateway(I know, makes no sense). The second, ended being a wiring
issues, tip / or ring was bridged to another line or something like
that.
