[cisco-voip] CallManager 4.0 Integration with Outlook problem

Hi All,
I've been unsuccessful in trying to get "Outlook Dialing"
working with CallManager 4.0(2a)sr1a and Outlook 2000/Outlook 2002.
I have followed the "Cisco CallManager TSP Integration with Microsoft
Outlook" instructions (URL below) with no luck and to be honest I've
run out ideas on what to test.

I made sure that the PC being used is free of any previous Cisco TSP
installation. I achieved this by following the instructions in the
attached file (ciscotsp.txt) for CiscoTSP 4.0.
Following the above instructions I enabled my test user for "CTI
Application Use" and I installed CiscoTSP version 4.0(0.2203).
The attached file (ciscotsp.txt) suggests that the "Cisco TAPI Wave
Driver" should be uninstalled and then re-installed to make sure that
the CiscoTSP is installed properly. It also suggests that TSP
operation can be verified by running the Windows Phone Dialer
(Windows\System32\dialer.exe).
In the Windows Phone Dialer Options, I was able to select a "Cisco
Line" for Phone Calls and therefore I was then able to make a phone
call (via a Cisco 7970G) using the Windows Phone Dialer.
The above suggests to me that the CiscoTSP successfully authenticated
with CallManager. However, when I select a contact from the Outlook
Contacts folder, I am not able to select a "Cisco Line" from the
"Connect using line" drop-down box under the "Dialing Options"
I would greatly appreciate it if any of you can suggest a possible fix.
