Is it a new install or just stoped working?
Are you using CM 3.3(5)sr1?

The "Bad Address" message is displayed when the Softphone is signaled
to play reorder tone.  Check the normal things you would for an IP
Phone; CSS, codec or other call setup issues that would fail the call,
etc.
