[cisco-voip] 2 DIDs incoming call automatically forward to voicemail and outside number

(1) make sure change notification is working - if you reset the phone  
from ccmadmin does it reset?
if not, restart Cisco Database Layer Monitor service on the publisher  
and try again

Once phone is successfully resetting, then delete the lines from the  
system completely and then re-add them with all forwarding destinations.

Otherwise, restart ccm.exe on the node in the cluster that is  
processing the call.
