[cisco-voip] echo issues with system

Can someone give me some ideals on the best way to trouble shoot echo.

 

We have a CM 4.1.3 and unity 4.0  

Core sw 3750  and  daisy chained are 2 3560's  (trunk dot 1q) 

We have vlan for voice and one for data 

 

Switch port are configured like

 

mls qos trust device cisco-phone

 mls qos trust cos

 auto qos voip cisco-phone
