Could it be that it needs to go out the gw as international rather then national?

So when I look in call manager,  my route pattern is 9.011!  discard predot
and that it.  Nothing is set on the route list/ group.   Now on cm gw config
I have 

Outbound calls
Calling line = default 
Calling party sel = originator
Called party ie num = cisco
Calling party ie number = cisco
Called number plan = isdn
Calling number plan = isdn
Number of digits to strip = 0

You need to look in CM not at the gateway.  The gateway only sends  
what CM tells it to.
