Here I had the same issue, it was madening to go through the voice
router tweaks daily for about a week or so to try to figure it out,
finally a TAC call was placed for it and it they said that dsp versions
over 4.4.13 were affected by this bug. They suggested as well to
downgrade our dsp firmware to 4.4.12.
After that, everything was smooth and just worked as it should with our
settings we had in place.

echo cancellation coverage: 32ms
thats what we have now and it all seems to work just fine. Occasional
echo's here and there but that can be expected on where the call is
originating from I guess.

That's interesting. I had an IP phone user hears echo on calls to the
PSTN issue as well. I tweaked the voice port and had no success. It
got worse like James mentioned before. Then found that my DSP firmware
4.4.708 had a bug against it. This is what comes with the 12.3.14 T7
IOS (and many other versions I would assume). I upgraded to dsp
firmware 4.4.19 and it fixed my problem.
