There is no provision within CCM to insert pauses into the dialed 
string.  If you were to use H.323 dial peers instead of MGCP you could 
manipulate the digits and insert commas (2 second pause for each comma 
I believe) into the dialed string.  So, you could still use the second 
pattern, without the additional 9, and have CCM send the call via H.323 
to the GW.  The matched dial peer can then prefix the number of commas 
needed for the delay to work.
