What we do iis get a RJ-21 to RJ-21 cable and hook it up to a RJ-11
patch panel with RJ-21 connector on the back. It gives us 25 ports
of rj-11 that we can patch directly into the cat 5 or cat 6 run
(rj-11's center themselfs into rj-45 ports and work perfect)
If the existing cable is 110 blocks then I like to take a rj-21 cable
and punch it down onto a 110 block as well to make cross connecting
easy.
Like said in the other posts, Greybar will have this equipment or any
other telecom supply store
