[cisco-voip] Stable IPCCx version

A week ago I would have said 4.0.4SR1.. but i've had more problems in the
past week than the other 2 versions we've been on put together. All my JTAPI
call control groups just disappeared this afternoon while I was trying to
sort out another problem... fun times.
