[cisco-voip] Issues with some calls into unity after userenters *

Ended up getting TAC involved and we found that we had the following
command enabled on the MGCP Gateway:

'mgcp dtmf-relay voip codec all mode nse'

After removing this command, we could enter '*'.  Prior to removing
the command we were seeing a double '*' come through to unity causing
the call to hang up.

We added the command:
  'mgcp dtmf-relay voip codec all mode out-of-band'
and things are working fine.
