[cisco-voip] Lock phone / Login to extended CSS

You could implement this using the Line/Device approach described in the 
SRND:

the device has permission to call everywhere, but the logged out device 
profile uses block patterns to prevent reaching specific numbers.  when 
user logs in with their profile, they get a different (or no?) block 
patterns with their CSS.
