I messed with it a bit in CME but haven't had much luck getting
translation-profiles applied at the ephone-dn. I assume the same
issues would be seen setting them globally in the call-manager-
fallback config.
The definition of the transfer-pattern command implies that it only
controls the digits you are allowed to transfer to. By default the
system only allows you to transfer to other IP phones. In SRST I
assume this means that 11 digit transfers would work by default.
This would lead me to believe your current config of the 4-digit
transfer-pattern and the profile applied globally would work.

Have not personally tried calling a alias # from a IP phone in
fallback, so not sure if that will work for calls from IP Phones
offhand. It is meant for incoming calls.

No, I would like to be able to do a 4 digit transfer to another phone at
the same site. We use 11 digit extensions but mask them. Under normal
conditions the users can dial/transfer using 4 digits to other phones at
their site. I am hoping to duplicate this functionality under SRST.

The POTS dial-peer would need a destination if you want to use it as
an outbound dial-peer (which you do). Your goal here it to allow
people in SRST mode to dial 4 digits and route via the PSTN to other
sites correct?

I haven't played with the transfer-pattern config too much so I'm not
sure what affect that would have on this behavior.
Have you tried applying the translation-profile to a pots dial-peer
with a destination-pattern of 2...$?
How about running a 'debug voice translation' to see if it's getting
applied?
