[cisco-voip] CM 5.1 / IPCCx4.5

Ran into a couple of problems where DMA was spitting out errors like it was
no tomorrow...

So, you doing an upgrade to 5.04? If you can, get the 5.1.1 DVD and use the
5.1.1 DMA...
