[cisco-voip] IPCC agents randomly dis-associate & require re-association

I've run into the same problem with the following combos:

IPCC Enterprise 4.6.2 + CCM 3.3 + active directory
IPCC Enterprise 6.0 + CCM 4.1.3 + active directory

Had the following things happen:
-some devices will disappear from being associated to a user
-all devices will disappear from being associated to a user
-devices will lose their line process and still be listed as associated, 
but act otherwise

Have had multiple TAC cases and provided more logs/traces than I care to 
think about - never got any resolution or even an acknowledged bug.
